ASSIGNMENT OF RESPONSIBILITY AND ACCOUNTABILITY FOR SAFETY (Top)

Managers

1. To provide information, instructions, and assistance to all supervisory staff in order to protect the health and safety of all our employees.
 
2. To understand and enforce our safety policy, as well as the occupational health and safety legislation.
 
3. To provide all supervisory staff with an understanding of our safety program as well as relevant occupational health and safety legislation.
 
4. To provide all supervisory staff with proper, well maintained tools and equipment, plus any other special personal protective devices which may be required.
 
5. To provide ongoing safety education programs and approved first aid training courses as required. 

6. To monitor departments and projects and hold them accountable for their individual safety performance.

Supervisor/Foreman

1. To know and apply the firm's safety policy and relevant occupational health and safety legislation. 

2. To ensure that all employees are educated to work in a safe manner and that they use all protective devices and procedures required by this firm and by legislation to protect their health and safety

3. To advise all employees of any potential or actual dangers and how to isolate, prevent or remove them.
 
4. To arrange for medical treatment as required, in the case of injury or illness including transportation to a doctor or hospital as necessary. 

5. To report all accidents immediately, to investigate all accidents fully, and to advise management on how to prevent similar accidents in the future. 

6. To carry out regular inspections of the work place to ensure a safe and healthy environment.

Employee

1. To read, understand and comply with this firm's safety policy, safe work practices, procedures and rules.
 
2. To wear the safety equipment and personal protective devices and clothing required by regulations and his/her employer. 

3. To notify his/her supervisor of any unsafe conditions or acts that may be of danger to other workers or himself/herself. 

4. To report all accidents and injuries to his/her supervisor as soon as possible. 

5. To take every reasonable precaution to protect the safety of other workers and himself/herself.

OBLIGATIONS OF EMPLOYEES AND WORKERS (Top)

Every worker shall, while engaged in an occupation,

a) take reasonable care to protect the health and safety of himself and of other workers present while he is working, and

b) co-operate with his employer for the purposes of protecting the health and safety of himself, other workers engaged in the work of the employer and other workers not engaged in the work of that employer but present at the work site at which that work is being carried out.

EXISTENCE OF IMMINENT DANGER (Top)

No worker shall

a) carry out any work if, on reasonable and probable grounds, he believes that there exists an imminent danger to the health or safety of that worker.

b) carry out any work if, on reasonable and probable grounds, he believes that it will cause to exist an imminent danger to the health or safety of that worker or another worker present at the work site, or

c) operate any tool, appliance or equipment if, on reasonable and probable grounds, he believes that it will cause to exist an imminent danger to the health or safety of that worker or another worker present at the work site.

PROCEDURE BASICS FOR ALL EQUIPMENT (Top)

Prepare For Safe Operation

1. Study the operator's manual or receive complete instructions from an operator who is competent in the operation of the particular machine. Learn the operating, maintenance, capacities and limitation of the machine. 

2. Learn the location and function of all controls, indicators, safety and warning devices on the machine. 

3. Know and check protective devices on the machine. Have missing or damaged protective devices replaced or repaired by authorized personnel.
 
4. Inspect tires and/or tracks for inflation, wear, tension and damage. 

5. Know your working area: - Condition of road, bridges - Direction of travel - Holes, obstructions, mud, ice - Utility locations - Traffic conditions - Rules of the work site 

6. Walk around your machine for a complete visual check.
 
7. Unloading: Know the correct procedures for your machine. Always unload on level surface, use proper ramps, keep personnel clear. 

8. Fire Prevention: All engines to be turned off for refueling. Do not refuel while smoking or when flame or sparks are present. Batteries contain explosive gas. Follow manufacturer's instructions for service, charging and boosting. Remove all trash, etc. from machines. Check fuel, oil and hydraulic lines for leaks. Check electrical wiring for wear and damage.
 
9. All machine operations to conform to Occupational Health and Safety.

Starting Equipment

1. Check around and under machine to ensure all is clear, warn any persons nearby that you are starting. 

2. Know the correct starting procedure: - Position yourself properly - Review controls, gauges, warning devices - Make sure park brake is engaged and shift to neutral - Sound alarm and start engine per manufacturer's manual 

3. If boosting is required, follow manufacturer's instructions. If starting fluid is required, read instructions, check if suitable for machine.
 
4. It is the operators responsibility to check machine systems and shut down if not proper. Test steering, right and left while moving slowly. Listen for unusual noises, feel for unusual vibrations.

GENERAL RULES (Top)

G & J Parking Lot Maintenance Ltd. accepts it full share of responsibility for the Health and Safety of its employees and believes that all personnel associated with its construction projects are committed to performing their trade activities in a conscientious, safe and productive manner.

We are also aware that in order for the Safety Management System to positively impact the company, that the rules and regulations we subscribe to within the company, and those set out by regulatory bodies by which we abide, be complied with in all tasks related to our daily working activities. Positive contributions to the Health and Safety Program will be recognized and acknowledged by management.

When individuals fail to comply with our Safety Policy and Rules and Regulations, they threaten their own well being and that of their co-worker as well as the public at large and their behavior, whether positive or negative, will impact G & J Parking Lot Maintenance Ltd. long after they have left the company.

G & J Parking Lot Maintenance shall hold the individual, in the case of its employees and the subcontractor where applicable, responsible and accountable for transgressions in relation to our company rules and those of regulatory bodies.

Personnel who by way of deed, are negligent in the discharge of their duties will be subject to disciplinary measures proportionate to the gravity of the offense as stipulated and categorized in the following pages.

Infractions Of Rules And Regulations As Follows Will Subject The Offending Employee To Immediate Suspension And/Or Dismissal Without Recourse, Pending A Review By Management.

(a) Possession or use or sale on the job of intoxicating beverages or unauthorized drugs.

(b) Theft, vandalism or any other abuse or misuse of company property.

(c) Activities which contribute directly or indirectly to or may cause injury to an employee.

(d) Driving While Impaired (DWI) offence.

(e) Falsifying documentation or impeding investigations.

Infractions Of Rules And Regulations As Follows Will Subject The Offending Employee To The Following Disciplinary Measures:

  • One recorded verbal warning
  • One written warning followed by a disciplinary suspension of one week without pay
  • A repeat offender shall be discharged from employment

G & J Parking Lot Maintenance Ltd. employees shall:

(a) Perform all work in accordance with safe work practices and your supervisor's direction;

(b) Safety glasses, safety boots and vests may be needed as required by project conditions;

(c) Hard hats to be worn as per Occupational Health and Safety Regulations;

(d) Obtain first aid treatment promptly for an injury;

(e) Report all unsafe acts and conditions, including accidents, injuries, "near misses", regardless of their nature to the supervisor;

(f) Operate all vehicles and mobile equipment in accordance with site rules and highway regulations;

(g) Explosive/powder actuated tools shall be used only by persons who have been instructed and trained in their safe use;

(h) Ensure that all compressed gas cylinders are secured in an upright position;

(I) Riding on equipment is prohibited. No person shall ride any hook, hoist or other material handling equipment which is used strictly for handling material and not specifically designed to carry riders;

(j) Horseplay, fighting and possession of firearms are strictly forbidden on the job and constitute grounds for dismissal;

Infractions Of Rules And Regulations As Follows Will Subject The Offending Employee To The Following Disciplinary Measures:

  • One recorded warning or reprimand.
  • Two written warnings.
  • Dismissal pending review by management.

(a) Power tools shall be operated only by authorized personnel, with guards furnished by manufacturer "in place";

(b) Hand tools shall not be used for any purpose other than that intended. All damaged or worn parts shall be promptly repaired or replaced;

(c) Welding and burning operations shall be carried out only by authorized personnel with appropriate individual protective equipment;

(d) Only those tools that are in good repair, with all guards and safety devices in place shall be used;

(e) Every worker shall keep his/her work area neat, clean and orderly;

(f) Running is not permitted anywhere, except in the case of extreme emergency;

(g) Shorts and muscle shirts shall not be substituted for appropriate construction outerwear, i.e., long pants, 4" long sleeved shirts.

Management Also Reserves The Right To Enact Any Automatic Dismissals Dependent On The Severity Of The Infraction And The Determination Of Deliberate Intent.

PERSONAL PROTECTIVE EQUIPMENT POLICY (Top)

G & J Parking Lot Maintenance Ltd. is committed to the safety and well being of all personnel working on, or visiting our project sites. We build safety into our project through Hazard Assessment & Personnel Training. All employees, subcontractors and visitors are required to wear their Personal Protective Equipment (PPE) should an unforeseeable breakdown in our incorporated engineering and administrative controls occur.

CSA approved Hard Hats and Safety Footwear may not prevent an accident from occurring, indeed this is not their purpose. They are designed to prevent injury and shall be considered the minimum standard of Personal Protective Equipment on all G & J Parking Lot Maintenance Project Sites where required by Occupational Health & Safety.

It is our workers responsibility to protect themselves and ensure that the PPE they are using is kept in a good state of repair. The superintendent's responsibilities is to ensure and enforce its daily proper use.

Specialized PPE, other than the minimum standards mentioned above, shall be provided for by G & J Parking Lot Maintenance Ltd. and our employees trained in their use. Safety glasses, hearing protection and breathing apparatus are but three examples.

G & J Parking Lot Maintenance Ltd. acknowledges their Superintendents responsibility to maintain a safe work site and ensuring this policy is adhered to.

Where questions, in regards to what is considered adequate PPE or when it is to be worn, arise the Superintendents decision shall be considered an integral part of this policy, specific only to the project he is supervising.

PERSONNEL FAILING TO COMPLY WITH G & J PARKING LOT MAINTENANCE LTD. P.P.E. POLICY SHALL BE DENIED ACCESS TO THE PROJECT SITE.

PERSONAL PROTECTIVE EQUIPMENT POLICIES (Top)

All employees must understand that the use of personal protective equipment is the last line of defense and should only be used when Engineering and Administrative Controls cannot be employed.

The minimum acceptable standards for employees, as well as visitors to the job site are as follows: 1) C.S.A. Approved steel-toed, steel shanked, safety footwear appropriate to the working environment. 2) C.S.A. Approved Class C head gear (hard hats) must be worn. 3) Minimum requirements are to be long pants and shirts with a minimum of a 4" sleeve.

The above are the minimum personal protective equipment to be worn at all times.

The following personal protective equipment will be required but is job specific. You will be informed by your Site Superintendent prior to Job Start-up as to the specific PPE required for Standard Job Procedures. Any other Personal Protective Equipment to be used should be researched in the available Material Safety Data Sheet binder prior to commencement of work on the job site. It is your responsibility to inform your Site Superintendent if you are not able to locate the MSDS's for the specific procedure to be performed.

You should also have the following PPE as it will be required for some operations. Your Site Superintendent will advise you when it is required.

1) C.S.A. approved safety glasses or C.S.A. approved frames with side shields. In the event of employees requiring prescription lenses, these must be treated with a form of hardening to make them impact resistant. Also mono-goggles or face shields will be required for some operations.

2) Gloves must be worn and will be supplied. Check with your Supervisor and available MSDS's as to the type required in your job function.

3) Employees working in confined spaces or in poorly ventilated areas will be clean shaven so as to effect a seal in the event that breathing apparatus is required.

4) No head wear except hair nets and welder type caps will be permitted under hard hats.

5) All workers will be trained in the proper use of Personal Protective Equipment.

6) All Occupational Health and Safety Governmental Regulations will be met and if possible exceeded in the proper operation and maintenance of Safety Equipment and Personal Protective Equipment.

7) All employees will be required to follow directives on the use and administration of Personal Protective Equipment.

8) All employees will be trained in the Legislated Workplace Hazardous Materials Information System (WHMIS) and will be required to recognize WHMIS labeling on all Hazardous Materials. Check that the required labels are in place. If not, notify the supplier. If the labels have been accidentally removed or become illegible, they must be labeled with work site labels by the employer. When a hazardous material is transferred to another container, then that container must also be labeled.

9) Training will also be supplied with WHMIS on how to determine Material Safety Data Sheets.

10) When wastes cannot be disposed of safely by conventional methods, such as landfill sites, sewage systems or burning, then arrangements must be made for proper storage and collection. They must be then transported to an approved disposal site for HAZARDOUS waste.

11) Proper hearing protection must be used in areas where there is an excessive noise hazard. This is the leading cause of hearing loss and exposure to this type of environment over long periods of time without protection will cause irreparable hearing loss. Hearing protection can be either C.S.A. approved muff or earplugs.

12) Respiratory Protective Equipment must be worn in oxygen-deficient environments or when there is a hazard of toxic or disease producing contaminates. These can be in the form of dust, vapors, fumes, smoke, gas, or mists. Government Approved Respiratory Protective Equipment must be used and you will be trained in the proper use and selection of this equipment.

13) All Protective Equipment must be maintained and stored in a clean, dry place. Anyone who refuses to use the Proper Protective Equipment will be dealt with under the section of Disciplinary Measures.

Remember the use of this equipment is for your health and safety and that of your co-workers. Not only is it required by G & J Parking Lot Maintenance Ltd. that these procedures are followed, but it is also required by the law.

As well, it is the effects of not using this equipment which is the leading cause of most Occupational Health and Safety Diseases such as hearing loss, long-term chronic respiratory problems and cancers such as silicosis and asbestosis to name a few. The effects of these health hazards may not be apparent for 10 to 20 years.

INVESTIGATIVE PROCEDURE IMMEDIATELY FOLLOWING AN INCIDENT (Top)

1) One individual shall TAKE CHARGE OF THE SITUATION.

2) Call for assistance, notify Site Superintendent.

3) Make the area safe.

4) Attend to the injured needs first.

5) Where required get Medical Aid.

6) Seal the area, to preserve physical evidence and to prevent any possibility of the incident reoccurring. The scene of the incident/accident shall not be disturbed unless activities involving rescue work or prevention of further system failures, necessitate this action.

7) The Site Superintendent will notify the safety department and under its direction proceed with the investigation.

8) If an investigative team is formed one individual will delegate specific areas of responsibility to each team member. The team leader shall be responsible for coordinating all activities relative to the investigation and for compiling the investigative report.

9) The investigating team or individual shall, analyze all pertinent information to determine the cause of the incident including witness statements if applicable. Interview witnesses and record their statements.

10) Take photographs of and collect and preserve physical evidence.

11) Examine materials, systems and equipment which were involved in the incident.

12) Review all applicable in place safety measures in order to determine and identify deficiencies in planning or procedure.

13) Determine corrective measures and implement same. Distribute the results of the investigation to all areas within the company and forward to regulatory bodies when and where applicable.

EMERGENCY RESPONSE POLICY (Top)

It is the policy of G & J Parking Lot Maintenance Ltd. to have in place on all its project sites a viable emergency preparedness plan. Said plan shall be an integral part of all job preplanning processes.

In the event of an incident or accident, the first priority shall always be the health and safety of our employees. With this criteria in mind, it shall be the responsibility of the site Superintendent and relevant Project Manager to identify emergency requirement needs and liaise with the safety of department to ensure that these concerns are addressed.

First Aid personnel and equipment of the numbers and type required by the relevant legislation shall be available on all G & J Parking Lot Maintenance Ltd. job sites. In cases requiring specialty emergency response equipment our personnel, where appropriate, shall be trained in its use.

Emergency contact numbers shall be prominently displayed in all job shacks and all personnel oriented to the location of this information and instructed in the use of any specialty communication device used in relation to a state of Emergency.

The Project Manager and Superintendent shall remain responsible for reviewing the Emergency Procedures associated with their project sites and shall in conjunction with the Safety Coordinator, review emergency measures enacted in the event of a downgrading incident. The review process will reveal areas of strengths and deficiencies of our plans and allow remedial measures to be implemented.

In all cases of Emergency Response inquiries by the media shall be directed to the office of the General Manager of G & J Parking Lot Maintenance Ltd.

DEFINITIONS OF EMERGENCY LEVELS 1,2,3. (Top)

EMERGENCY LEVEL.1

  • Accidental death;
  • Dismemberment;
  • Destruction of Company property in excess of $10,000.00

EMERGENCY LEVEL.2

  • Destruction of public property and/or environmental damage;
  • Injuries which require loss of work day investigations

EMERGENCY LEVEL.3

  • Situations, where the potential for downgrading incident exists or imminent danger to G & J Parking Lot Maintenance Ltd. personnel, equipment, the environment or the public is recognized.

EMERGENCY PROCEDURES In all cases of worker injury, the FIRST RESPONDER on the scene shall: 

1. Take charge of the situation.
 
2. Make the area safe.
 
3. Call for help. Do not leave the casualty, send someone to notify the superintendent.
 
4. Assess the casualty's injuries.

5. Treat for life threatening conditions, at the location he is found, if possible.
 
6. Ensure medical aid is on the way.

Level I Emergency Procedures - Death and/or Dismemberment 

1. Initiate First Responder Procedures. 

2. The superintendent shall ensure that the First Responder Procedure has been activated. This is to assure the safety of all remaining site personnel. 

3. If conditions warrant he shall issue a cease work order to all personnel. If the situation poses no further threat to the individual on the site, he shall secure the Site of the Accident to preserve physical evidence. 

4. Contact R.C.M.P. and O.H.& S. and the office of G & J. 

5. Direct media inquiries to the office of the General Manager of G & J Parking Lot Maintenance Ltd. 

6. Notification of family shall be performed by the office.

Level I Emergency Procedures - Property Damage Over $10,000.00 

1. Initiate First Responder Procedures. 

2. Seal the area. 

3. Contact authorities. 

4. Inform office.

Level 2 Emergency Procedures - Destruction of Public Property or Environmental Damage, Accidents Requiring Loss of Work Days Investigations 

1. Initiate First Responder Procedures. 

2. Contact the offices of G & J after consultation with Head Office, the Owner of the project shall be notified. 

3. Contact appropriate authorities.

Level 3 Emergency Procedures - Situations where there Exists a Potential Danger to Personnel, Equipment, the Public or the Environment 

1. Call out a warning. Halt work. Notify Superintendent. 

2. The Superintendent shall take measures to immediately eliminate the potential hazard, if the activity or condition is attributable to G & J activities on the project site. 

3. Where events, which are beyond the control of G & J Superintendent's may adversely impact the health and safety of those present on the project site, the Superintendent shall contact the office of G & J Parking Lot Maintenance Ltd. as soon as possible. 

4. The project manager shall after consulting with the project superintendent declare a state of emergency to exist and notify the proper authorities.

I have received a copy of the G & J Parking Lot Maintenance Ltd. Safety Manual and understand that in accepting employment with G & J Parking Lot Maintenance Ltd., I am expected to abide by these rules, and further understand that a violation of these rules or failure to perform my work in a safe manner will be cause for my termination.
